Lab Mount Sheets
Mount sheets are an integral part of the daily operation of hospitals, laboratories, and other medical organizations. These forms are used to collect patient data and organize it for effective use. The mount sheet may be used simply as a document carrier or as a document carrier for microfilming purposes. Mount sheets usually have 5 holes punched at the top or to the side so that they can be easily mounted to a clipboard or folder for easy access. The standard size is 8.5 x 11". They are also made with a pressure-sensitive tab or adhesive tape (vertical or horizontal) to attach a report to supporting documents.
